How does insurance work at Foothills Acupuncture?

Foothills Acupuncture in Lakewood, Colorado, accepts insurance benefits for acupuncture treatment. If your health insurance plan includes acupuncture coverage, those benefits can typically be used toward eligible services at our clinic. We are currently in network with UnitedHealthcare, Anthem Blue Cross Blue Shield, and TriWest Healthcare Alliance. Because coverage varies by plan, we encourage you to contact us so we can help verify your acupuncture benefits before your first visit.

What if my Insurance is Out of Network with Foothills Acupuncture?

Even if Foothills Acupuncture is out of network with your insurance company, you may still be able to use your acupuncture benefits toward treatment. To simplify the process, we provide complimentary out-of-network claim submission, eliminating the need to prepare and submit superbills on your own. This service is designed to help you access any available out-of-network benefits while making the claims and potential reimbursement process as clear and convenient as possible.

Does Foothills Acupuncture accept FSA and HSA payments?

Foothills Acupuncture accepts both Health Savings Account (HSA) and Flexible Spending Account (FSA) payments for eligible treatments. We can also provide a detailed receipt containing the information needed to document your treatment or submit a reimbursement request to your plan administrator. Because individual HSA and FSA requirements may vary, please contact us if you have questions or would like additional information before scheduling your appointment.

Does Foothills Acupuncture accept workers’ compensation claims?

Yes. Foothills Acupuncture accepts approved workers’ compensation claims for acupuncture treatment. Because coverage and authorization requirements vary by claim, treatment may need to be approved by your workers’ compensation insurance carrier or claims adjuster before care begins. Please contact our clinic with your claim information, adjuster’s contact details, and any available authorization documents. We will review the information, help clarify the next steps, and determine whether treatment can be provided under your claim.

Foothills Acupuncture’s Insurance Policies

At Foothills Acupuncture, we want to help patients use their available insurance benefits successfully while reducing the possibility of unexpected out-of-pocket expenses. As a courtesy, we submit eligible claims to participating in-network and out-of-network insurance plans. However, verification of benefits does not guarantee coverage, reimbursement, or payment by an insurance company.

Patient Responsibility

Insurance coverage, deductibles, copayments, coinsurance, visit limits, reimbursement rates, referrals, authorizations, and other requirements vary by plan. It is the patient’s responsibility to confirm that their plan includes acupuncture benefits and to understand their coverage and financial responsibility before receiving treatment.

Patients are ultimately responsible for all charges not paid by their insurance plan, including deductibles, copayments, coinsurance, denied claims, noncovered services, and amounts applied toward visit limits or other plan restrictions.

Claim Submission and Denied Claims

Foothills Acupuncture will submit eligible insurance claims and may attempt to correct and resubmit a denied claim up to three times. If a claim remains denied, unpaid, or otherwise unresolved after these attempts, the remaining balance will become the patient’s responsibility and will be billed directly to the patient.

Credit or Debit Card Requirement

A valid credit or debit card must be kept on file for Foothills Acupuncture to submit insurance claims on a patient’s behalf. If a patient-responsibility balance remains unpaid, we will make up to three attempts to notify the patient and collect payment. If the balance is not resolved after the third attempt, the card on file may be charged for the outstanding amount.

Additional Services and Products

Herbal formulas, specialty supplies, recommended products, and services not covered by a patient’s insurance plan are billed separately and remain the patient’s responsibility. Prices, insurance participation, and service availability are subject to change.

Patient Acknowledgment

By receiving treatment and authorizing Foothills Acupuncture to submit insurance claims, the patient acknowledges and accepts financial responsibility for any deductible, copayment, coinsurance, noncovered service, denied claim, product, or other balance not paid by the insurance plan.
