Foothills Acupuncture Payment Policies
At Foothills Acupuncture, we strive to make our pricing and payment expectations clear so that patients can make informed decisions about their care. Patients are responsible for understanding the cost of their services and paying all balances according to the policies below.
General Payment Policies

By scheduling or receiving services from Foothills Acupuncture, the patient acknowledges and accepts financial responsibility for all services, products, fees, and balances not paid by an insurance plan or another authorized payer.

Payment is due at the time services are provided unless prior arrangements have been made or an eligible insurance claim is being submitted.
Foothills Acupuncture accepts all major credit and debit cards, including tap-to-pay, chip insert, and PIN debit transactions, as well as Apple Pay, Google Pay, HSA and FSA payments, cash, and personal checks.

Patients using insurance or participating in a treatment program with scheduled payments are required to maintain a valid credit or debit card on file. Patients enrolled in automatic payments must also complete and sign the applicable card-on-file and recurring-payment authorization forms. All other patients may choose to keep a card on file for convenience.
The card on file may be used to collect authorized charges, including:
Deductibles, copayments, and coinsurance
Services not covered by insurance
Balances remaining after insurance processes or denies a claim
Late-cancellation or missed-appointment fees
Payment-plan installments or other charges authorized by the patient
Patients are responsible for keeping their payment information current.

Patients will be notified when an outstanding balance is due.
Foothills Acupuncture may make up to three attempts to contact the patient and collect payment. If the balance remains unresolved after the third attempt, the card on file may be charged for the outstanding amount.
Future appointments may be postponed until overdue balances have been resolved or an approved payment arrangement has been established.

Payment plans are available for qualifying treatment programs, microneedling programs, and eligible outstanding insurance balances. Approval must be obtained before the payment plan begins.
A valid credit or debit card must remain on file throughout the payment-plan period. By enrolling, the patient authorizes Foothills Acupuncture to charge each installment automatically according to the written payment schedule provided at enrollment. The schedule will identify the amount and timing of every payment.
The initial installment is due when the program is purchased. All remaining installments will be charged automatically to the card on file. Treatment programs must be paid in full before the final included treatment is received.
If a patient extends or pauses treatment beyond the anticipated completion period, the established automatic-payment schedule will continue until the balance has been paid in full. Pausing treatment does not pause or extend the payment schedule unless Foothills Acupuncture approves a revised arrangement in writing.
Failure to complete scheduled payments may result in the suspension of program benefits and future appointments until the balance is resolved.

If an automatic payment is declined, the patient will be notified and asked to provide updated payment information. Program benefits and future appointments may be suspended until the missed payment has been successfully processed or another arrangement has been approved.
If a payment remains unresolved, the outstanding amount will continue to be the patient’s responsibility and may be charged to an updated card on file with the patient’s authorization. Severely delinquent accounts that remain unresolved after reasonable notice and collection attempts may be referred to a third-party collection service.

Requests to change a payment date must be made before the scheduled charge and are subject to approval.
Canceling or revoking automatic-payment authorization does not cancel the patient’s financial obligation for treatments, products, or services already received. Any unpaid balance will remain due through another accepted payment method.

We kindly ask that patients provide at least 24 hours’ notice when canceling or rescheduling an appointment.
Missed appointments and cancellations made with less than 24 hours’ notice are subject to the following fees:
First occurrence: 50% of the scheduled treatment price
Second occurrence: 100% of the scheduled treatment price
Third and subsequent occurrences: 100% of the scheduled treatment price; the patient may also be restricted from scheduling future appointments
Cancellation fees are the patient’s responsibility and cannot be billed to insurance.

Herbal formulas, specialty supplies, skin-care products, and other recommended products are priced separately unless expressly included in a treatment program.
Due to health and safety considerations, opened or dispensed herbal formulas and products are nonreturnable and nonrefundable.
Unopened products may be returned within seven days of purchase, provided they remain unused, undamaged, and in their original packaging and are in a condition suitable for resale. Herbal formulas, custom-ordered items, opened products, and refrigerated or temperature-sensitive products are not eligible for return.

Services that have already been provided are not refundable. Payments for individual stand-alone treatments and therapeutic tools are final and nonrefundable.
Unused portions of prepaid treatment programs or packages may be eligible for a refund. Refunds are calculated from the amount paid for the program, minus the full à la carte price of each treatment already used. This calculation applies to all acupuncture, microneedling, cosmetic, and other prepaid treatment programs or packages.
For example, if three treatments from the $700 Reset Program have been used, the refund would be calculated as follows:
Reset Program purchase price: $700
One Initial Acupuncture Appointment: −$150
Two Follow-Up Acupuncture Appointments: −$200
Remaining refundable balance: $350
Any products already dispensed, promotional benefits used, complimentary services received, or other program savings may also be deducted at their full retail or à la carte value when calculating an approved refund. If the value of the treatments, products, and benefits already received equals or exceeds the amount paid, no refund will be issued.
Herbal formulas remain subject to the herbal-product refund terms stated in this policy. Eligible unopened and unused retail products may be returned within seven days of purchase. Opened products, used products, customized items, and therapeutic tools cannot be returned or refunded..

Treatment Program Payment Policies

Payment for a treatment program is due before or at the beginning of the program unless an approved payment plan has been established.
Program benefits apply only to the treatments and services specifically included with the purchase. Additional treatments, herbal formulas, products, specialty supplies, and other services are billed separately.
All program treatments must be used within 12 months of the original purchase date. Treatment programs are nontransferable. Unused treatments remaining after the expiration date are not eligible for a refund, transfer, or account credit unless otherwise required by law.
Program Cancellation
A patient may cancel a treatment program at any time before the program is completed or expires. There is no treatment-number cutoff. Any eligible refund is determined by the unused financial balance remaining after completed treatments and other benefits received are recalculated at their regular purchase-date prices.
Treatments and services already provided are not refundable.
Refund Calculation
Approved refunds are calculated using the following formula:
Refund = total amount paid − full purchase-date value of treatments used − value of products or additional program benefits actually received − outstanding fees or balances
Completed treatments are recalculated at the regular individual prices that were in effect when the program was purchased.
Products, complimentary services, or program-only discounts actually received or used may be deducted at their purchase-date value.
Program savings are not deducted separately. Recalculating completed treatments at their full individual prices already removes the program discount.
If the calculation results in a positive balance, that amount is eligible for a refund.
If the calculation results in zero or a negative balance, no refund is due.
A fully paid program will not be charged an additional amount solely because the full-price recalculation exceeds the original program price.
Refund Example
If a patient pays $700 for the seven-treatment Reset Program and completes four treatments regularly priced at $150, $100, $100, and $100:
Total amount paid: $700
Initial Acupuncture Appointment: −$150
Three Follow-Up Acupuncture Appointments: −$300
Approved refund: $250
The same calculation applies regardless of how many treatments have been completed. As more treatments are used, the remaining refundable balance decreases until no refundable balance remains.
Programs Purchased Through a Payment Plan
For programs purchased through a payment plan, the refund calculation is based on the total amount paid at the time of cancellation:
If the patient has paid more than the value of the treatments, products, and program benefits already received, the eligible difference will be refunded.
If the value already received exceeds the amount paid, the remaining balance becomes due. The amount due will not exceed the unpaid balance under the original program agreement, plus any authorized fees or finance charges.
Any amount due may be charged to the card on file according to the signed payment authorization.
Remaining automatic payments will be discontinued after the final account balance has been calculated and satisfied.
Approved refunds will be returned to the original payment method whenever possible.
